Measuring Cerebrovascular Reactivity: Sixteen Avoidable Pitfalls

چکیده
An increase in arterial PCO 2 is the most common stressor used to cerebral blood flow for assessing vascular reactivity (CVR). That CO readily obtained, inexpensive, easy administer, and safe inhale belies difficulties extracting scientifically clinically relevant information from resulting responses. Over past two decades, we have studied more than 2,000 individuals, with cervical pathology using as vasoactive agent oxygen-level-dependent magnetic resonance imaging signal surrogate. The ability deliver different forms of precise hypercapnic stimuli enabled systematic exploration flow-related changes. We learned effect on CVR particular aspects stimulus such partial pressure oxygen, baseline , magnitude, rate, pattern its change. Similarly, interpret response speed direction Finally, were able test whether falls into a normal range. Here, present review our accumulated insight 16 “lessons learned.” hope many these insights are sufficiently general apply range types -based perfusion metrics CVR.

Measuring cerebrovascular reactivity: what stimulus to use?
Cerebrovascular reactivity is the change in cerebral blood flow in response to a vasodilatory or vasoconstrictive stimulus. Measuring variations of cerebrovascular reactivity between different regions of the brain has the potential to not only advance understanding of how the cerebral vasculature controls the distribution of blood flow but also to detect cerebrovascular pathophysiology. While t...

Background & Aim: Preeclampsia is the major cause of maternal and perinatal morbidity and mortality. Transcranial Doppler(TCD) as a noninvasive procedure is used to assess hemodynamic changes in middle cerebral artery(MCA). Knowing the influence of normal pregnancy and preeclampsia on brain hemodynamics is essential for proper management of anesthesia, labor and vasoactive drug usage in the...
متن کاملCan cerebrovascular reactivity be assessed without measuring blood pressure in patients with carotid artery disease? .
BACKGROUND AND PURPOSE Conventional methods of assessing cerebrovascular reactivity (CVR) omit the influence of blood pressure (BP). This study demonstrates the significant influence of BP during the assessment of CVR in patients with carotid artery disease. METHODS In 56 subjects the CVR was bilaterally assessed by measurement of cerebral blood flow velocity change in response to inhalation ...
متن کاملCerebrovascular reactivity measured by near-infrared spectroscopy.
BACKGROUND AND PURPOSE The pressure reactivity index (PRx) describes cerebral vessel reactivity by correlation of slow waves of intracranial pressure (ICP) and arterial blood pressure. In theory, slow changes in the relative total hemoglobin (rTHb) measured by near-infrared spectroscopy are caused by the same blood volume changes that cause slow waves of ICP. Our objective was to develop a new ...
متن کامل82991 - Novel Method to Measure Cerebrovascular Reactivity
Introduction: Cerebrovascular reactivity (CVR), defined as a change in cerebral blood flow in response to a vasoactive stimulus, reflects the vasodilatory reserve capacity of cerebral vessels. CVR impairment has shown to be an important prognostic marker of several diseases including stroke. Several techniques have used to measure CVR but the lack of appropriate reproducible stimuli and noninva...
